Short Bowel Syndrome (SBS) Management to Improve Nutrition & Help the Gut Adapt

 

When a large part of the small bowel is removed, the route that nutrients take to get through the intestinal tract is shortened. It is no longer a normal pathway from the stomach to the end of the intestine, which can cause problems for the infant or child. In SBS patients, malabsorption of nutrients is common.

SBS management usually means special diets and nutritional supplements and may require nutrition through a vein to prevent malnutrition. This is known as parenteral nutrition (PN) and can be partial or total (TPN). Over time, many SBS patients on PN will be able to take more of their nutrition by mouth or feeding tube and can reduce or stop getting nutrition through their veins.

Over time, the remaining small intestine can adapt to be able to absorb more nutrition. Adaptation means growing more cells to help increase the gut’s ability to absorb nutrients and digest formulas and foods normally. During this period, some SBS patients develop food allergies.  If a child has a milk allergy, an amino acid-based formula like Neocate may be needed.

Neocate is different from other formulas and foods because it’s based on nonallergenic amino acids, the building blocks of protein.  Amino acids are easy for infants and children to absorb. Neocate has neither whole nor fragmented protein chains that can trigger an allergic response. Amino acid-based formulas, such as Neocate, are the most hypoallergenic type of formulas available.

Neocate is also made with a special fat blend. One-third of the fat in Neocate is easy-to-absorb MCT (medium chain triglycerides), which can help SBS patients with fat malabsorption. The other two-thirds of the fat in Neocate is LCT (long chain triglycerides) that needs to be broken down before being absorbed. Breaking down LCT may help SBS patients with gut adaptation. Always see a healthcare professional for routine nutritional monitoring. Serum levels of micronutrients, should be monitored by clinicians when amino acid-based formulas are used as a primary source of nutrition for patients with complex disease. This is extremely important with multiple diagnoses and intestinal disease, especially in combination with tube feeding and/or former preterm status. Neocate formulas are only for use under medical supervision.
